Cross-RHEC Community Health Worker’s Coalition Overview

Vision Statement: To achieve impactful progress toward regional and national recognition of the value of community health workers in community health promotion and health care delivery.

Mission Statement: To improve health equity and population health through the implementation and sustainability of CHWs nationally by addressing health disparities and social determinants of health, especially within underserved communities. This mission will be accomplished with the application of research, examining payment models and collaborating with key payers, providers, policy makers, community leaders, CHW leaders, and government agencies within the health sector.

  4. Identify available resources to support effective, scalable pilot demonstration programs for CHW employment, development and program sustainability.
